A collaborative project between The Independence Foundation, People Inc, and The Self-Advocacy Association of New York State.

Goals / parameters of the project:

• Must be at least 18, use a wheelchair, and be able to travel independently or with your own aide / assistant
• To provide as needed transportation when other resources fail.
• Effective transportation for individuals with physical disabilities

Exclusions:

• We can not provide transportation to places of employment and / or doctors appointments. Refer to Vesid and Medicaid for these.
• Intended for ages 18 - 55

Individuals using this program will:

• Call LaRenz at 548-0102 to schedule a ride.
  Rides available 7 days a week – most reasonable hours
  Designated calling hours to schedule rides:
  Tuesday, Thursday, Saturday between 5 – 7 PM

• We recommend that you apply for Getting There, Paratransit, and any other transportation options first. Other options and contact information can be found here
• Respond to a follow up phone call.
• Agree to use this program for reasonable and reputable outings.
• Agree to be ready on time for pick-up and return rides.

How the program will work:

1. You will fill out an application (Click here for application)
2. We will verify that you meet our program requirements and can travel independently.
3. You will be mailed a participation card and a letter of authorization for our program.
4. You may call Larenz to schedule rides.
5. You will participate in a follow up phone call.
6. If and when our funding is depleted – we will notify you that we can no longer provide rides.

Suggested donation for each ride:
$5 one way
$10 round trip
Payment can be mailed to the Independence Foundation at the above address. Block payments can be made to save on checks – for example – you can mail in $50 and we will keep track of your ride usage – after 5 rides you can send in another check. We will refund any of your remaining funds if and when our funding runs out and we must end this program. You will not be making any payments to the drivers.

All payment to Transportation Agencies will be made by The Independence Foundation for scheduled / authorized rides.
